You will be emailed a unique secure user document upload link to upload a copy of these compliance components prior to the commencement of the Program.

You cannot meet with your Young Adult until all compliance components have received and verified, AND your application has been cleared. Please note, this can take up to 4 weeks.

The following questions are required to meet the national reporting requirements of a Registered Training Organisation (RTO). Please answer them to the best of your ability.

Please list any previous leadership training and/or community involvement.

Some elements of the Program will be in-person, while others will be conducted online. This arrangement is subject to change based on unforeseen external circumstances.

This includes coaching sessions between the young adults and community coaches, full-group workshops, as well as workshops with smaller groups of students and coaches.

Acceptance into the Max Potential program as a participant is based on:

• Completion of the online application form and receipt of the following signed forms:
  • Signed Consent with Parent/Guardian Consent (Parent/Guardian Consent is only required if you are under 18).
• Applicant's commitment to the Max Potential program (as below)

I agree to commit fully to the Max Potential Program and will:

• Prioritise Max Potential in my schedule and attend all group activities and coaching sessions.
• Commit to personal growth and active development of my personal leadership skills.
• Fulfil the agreement I make with my coach and commit to developing an open, honest and transparent coaching relationship.
• Work to understand the MAXI principles and apply them to my life
  • Be clear about what success looks like.
  • Try new ways of thinking and doing things to achieve my goals.
  • Create space in my day to focus on new habit formation.
• Complete all requirements of the program within the required timeframe, including a
  • Creation of a Personal Expression film to be presented to the group and shared online via website and social media.
  • Development of a Community Service Project aimed at addressing a community need and reinforcing my personal learning.
  • Participation in the Celebration Event to celebrate my achievements.
• 'Push through' any challenges that arise to make the most of my Max Potential experience.

Acceptance into the program as a Community Coach is based on:

• Completion of this online application form.
• The submission of compliance components as specified below through the unique secure user document upload link which will be provided to you via email:
  1. Working With Children Check number
  2. A cleared National Police Check
  3. A Declaration for Child-related Workers Document
  4. Photo ID
• Attendance at all coaching training sessions and demonstration of coaching proficiency Important note: The Max Potential Program reserves the right to appoint as Community Coaches only those individuals who have fulfilled all program requirements and demonstrated an appropriate attitude and coaching competence following their training.
